Due by May 31, 2012
1. Annual Chapter Board Report - Template attached
2. Chapter By-Laws – submit to International Tricket Association immediately upon change
3. Chapter financial information – submit annuallyTemplate attached
4. Chapter Agreement – submit a signed copy annually
5. Conflict of Interest Policy – submit to International Tricket Association upon election of a new chapter board member.Template attached
6. D&O Insurance Policy Summary of Coverage – submit annually
7. Minutes from all Board meetings from the prior year annually by May 31st. All Board meetings shall include a report from the Chapter’s treasurer, which shall be included in the minutes of each Board meeting.

Due by November 30, 2012:
1. Chapter Tax Documents – submit annually to International Tricket Association when sent to IRS but no later than November 30. 

Due by December 31, 2012:
1. Participation Survey – submit annually
